Dr Rosenfeld's fee for the tubal reversal is $6,400 which covers pre-operative lab work, surgeon’s fee, anesthesia fee, operating room and post-operative care through the evening of the surgery. However, the cost of the On-Q pain pump (if requested) and the treatment of any unexpected medical complications are not covered by this fee.

The cost of any unexpected medical complications may be covered by your medical insurance, even if the tubal reversal is not.  Semen analysis is available for an additional fee.

Deposit

A $1000 deposit is required to schedule the tubal reversal surgery. The balance of $5,400 is due in full two weeks prior to the procedure. If you cancel your surgery at least two weeks before your scheduled surgery date, we refund $350 of the deposit to you. The deposit is non-refundable less than two weeks before surgery.

Insurance

We are providers for many managed care plans and health maintenance organizations and, for your convenience, we gladly file your insurance claims for you. If you do not find Dr. Rosenfeld in your Provider Directory please call as these plans change daily.

Financing

The Southwest Center for Microsurgical Tubal Reversals takes Visa, MasterCard, Discover, AMEX and checks. To pay online at any time, contact us with your payment information at 866.790.0095 and a receipt will be emailed to you within one business day.

Dr Rosenfeld also accepts CosmiCredit, an elective surgery loan provider, and MedChoice Financial, a patient financing provider.
